Guidelines for Documentation

Medical Disabilities

 

Students who have been diagnosed with a medical condition which limits a major life activity of the individual may qualify for accommodations through the Office of Accessibility and Accommodations. Students seeking accommodations for a medical disability must submit documentation from a qualified health care provider. The documentation should include the following information:

  • Diagnosis, date of diagnosis
  • Cause of condition
  • Impact or limitations
  • Medications
    • Side effects, if any, and the impact thereof on cognitive ability

 

To begin the registration process with our office:
  1. SUBMIT the following to A&A:
    1. Documentation (include ONE):

      1. An IEP or 504 plan that is less than 10 years old

      2. OR Physician's Statement to Determine Eligibility for a Functional or Physical Impairment   Word   PDF

    3. EIU/A&A Release of Information Form:   Word   PDF

  2. Then SCHEDULE an appointment with our office by email or phone (217) 581-6583.
