Accessibility and Accommodations recognizes that with the release of the DSM-V, that any previous diagnosis of Autistic Disorder, Asperger’s Disorder, and Pervasive Developmental Disorder not otherwise specialized prior to May 2013 are now recognized as Autism Spectrum Disorder.
Documentation:
An IEP or 504 plan that is less than 10 years old
OR Documenation that includes:
specific, current diagnosis per the DSM-V developed by a qualified mental health professional or a team determined educational diagnosis; which indicates the nature, frequency, and severity of the symptoms for which the diagnosis was based upon,
Prescribed medications, dosages, and schedules which may influence the types of accommodations provided, and
A clinical summary which:
Indicates substantial limitations to major life activities posed by the disability,
Describes the extent to which these limitations would impact the student within the university setting,
Suggests how the specific effects of the disability may be accommodated, and
States how the effects of the disability are mediated by the recommended accommodations.
