Guidelines for Documentation

Specific Learning Disabilities

A comprehensive evaluation/full and individual evaluation is the preferred documentation, which was utilized to determine eligibility for services through the RtI process. The documentation of this process will include record review, interviews, observations, and testing scores. Examples of these areas include:

  • Interviews
  • Observations of the student in specific, relevant settings
  • Error analysis of work samples
  • CBA/CBM/CBE’s
  • Progress monitoring
  • Results from local assessments
  • Behavior rating scales
  • Vocational assessments
  • Developmental, academic, behavioral, and functional life skills checklists
  • Standardized assessments
  • Achievement assessments

 

To begin the registration process with our office:
  1. SUBMIT the following to A&A:
    1. Documentation (include ONE or more):

      1. The most recent IEP;

      2. The most recent eligibility review;

      3. The student's most recent psychological evaluation;

      4. The letter from ACT stating the acceptable ACT accommodations for students with disabilities
