A complete application includes the attached application for services, student agreement contract, release of information authorization, and authorization to exchange or request information. Also, please provide a current copy of income documentation (you may contact our office directly at 217-581-7849 for an explanation of what is required). Please print, complete and mail your application and any supporting income documentation.
