Call the The Worker’s Compensation Hotline at 1-833-891-1372 after the injury/illness has occurred. If employee is unable to place the call, the supervisor may call.
Complete the following forms

Have witnesses complete the Witness Report of Injury form.
It is the employee's responsibility to keep the workers' compensation coordinator and his/her supervisor/department informed of his/her status and progress.

If the injury/illness involves restricted work activity, the employee needs to contact the workers' compensation coordinator and supervisor immediately and forward all appropriate medical documentation. Once this has been performed, the workers' compensation coordinator will contact the supervisor to discuss any modifications or restrictions regarding work activity. If the modifications or restrictions can be accommodated by the supervisor, the employee shall return to work. If the modifications or restrictions cannot be accommodated, the employee shall be off work until the modifications or restrictions can be accommodated or a return to work without restrictions is received, whichever occurs first.
Please contact the workers' compensation coordinator if you have restrictions on your work activity because of a work-related injury.
If the injury/illness involves days away from work, the employee needs to contact the workers' compensation coordinator and supervisor immediately and forward all appropriate medical documentation. If the injury/illness requires the employee to be off work for more than 14 days, a leave of absence form (FMLA) will need to be completed as well as the appropriate workers' compensation forms. The workers' compensation coordinator can assist the employee with this information and any questions.
